Skiers Beware – Moguls May Be Dangerous To Your Health… And What To Do About It

Listen up skiers.

There’s no easier way to damage your spine, discs and joints than  pounding down a ski slope at break-neck speed (No pun intended).   Especially with those little hills within the hills known as “moguls”.

During the typical skiing motion, there is tremendous compressive and rotational force absorbed by your low back, discs and joints.  This is sometimes called torque.  Add a few dozen moguls in and the forces begin to multiply and add up quickly.  Repetitive pounding of this sort can begin to take it’s toll.

So, before the season gets going full tilt, do a few of the following:

  1. Get to the gym and get in shape especially the core, thighs and calfs
  2. Do lighter very strict full range of motion exercise.  Keep the positive (contractile) portion of the exercise at 3 to 5 seconds with the negative (return to neutral) portion at 8 to 10.   You will have to keep the weight very light to perform reps like this.   Your muscles will get a heavy burning sensation… but you’ll be thankful for strength once on the slopes.
  3. Work on your flexibility every day.  Low back, hips, knees and ankles.
  4. Work on your balance
  5. Get any excess weight off
  6. Start your first few days of skiing lighter, less intense and less hours and work in to the full day, all out ski marathons to reduce risk of injury
  7. Don’t drink alcohol or smoke pot etec… on the mountain.  You’ll end up like Sonny Bono
  8. Don’t go on hills above your ski skill level.  If necessary, get some updated lessons from a pro.
  9. Go easier on the moguls if you’ve go previous back injuries or you’ve suffered with chronic pain so as to avoid more serious or season ending injuries

Otherwise, use your head and have a great time!!!

Back Or Neck Surgery Recommended? First Get The Facts…. Then Think Again

First let me say that I am not against surgery for the neck or back.  I am against surgery as a first line of care or defense for severe spinal pain.

And the facts support me.

Recent studies indicate that in a 5 year follow up, there was no statistically significant improvement in outcomes for those who did surgery… versus those who did absolutely nothing.  Now, I didn’t say these patients got better.  In fact, some got worse whether they did surgery or didn’t.

But the surgical patients did not do better. I find this disturbing considering the risk to the surgical patients including hospital induced infection, anesthesia reaction, possibility of negative surgical reaction or increased pain due to the procedure.

If find it more disturbing that patient’s actually opt for surgery because there is now a viable, safe, reliable and effective alternative available.

Studies as far back as 2003 by Dr. Dennis McClure, a neurosurgeon quite familiar with spinal surgery, indicate that patients recommended for surgery who opted for a protocol of non-surgical decompression therapy had a 92% success rate!.  This is a real eye-opener as in the past, these patients would have undergone extensive surgery exposing them to all the risks previously noted. Not to mention the sky high costs (around $154,000.00 including surgery, operating room suite, anesthesia fees, post-surgical rehab and hosptitilization according to the AMA).

Also interesting was the fact that those who had submitted to but failed  previous surgical “decompression” procedures known as “laminectomies” (whereby the a piece of the vertebrae called the lamina is removed) had a success rate of 79% with a protocol of non-surgical decompression.   Quite remarkable particularly as these patients typically have post-surgical scar tissue and muscle weakness due to the surgical “wounds”.

Conclusion:  If you’re thinking about surgery for back or neck pain…. Maybe you should think again and explore other options.

Can Medications, Pain Relievers Or Pain Management Procedures Be The Absolute Worst Thing For Serious Herniated Discs or Serious Neck & Back Pain? Part II

A brief word or two on epidurals and pain management procedures today.

First of all, like drugs, epidurals cannot and will not heal anything.  The reduce pain in about 50% of the cases.  However, pain relief is usually short term in many cases.

Number two.  It requires invasive needling of the spinal cord and “epidural space” or the little channel between the spinal cord itself and the internal bony aspect of the spinal canal.

Now, I am not against this procedure in and of itself.  But is not an answer to “discogenic” pain or pain originating from disc herniation, bulge and the like.  It can be helpful to patients who are in acute distress and cannot even be placed in a prone, lying down position on a spinal decompression machine to treat the disc properly or to those that have let the condition run wild and can barely move without pain.

But keep in mind that an epidural is just the use of a “liquefied”  medication that is injected in the area of the swollen or herniated disc so as to bathe it in anti-inflammatory medicine and calm it  down…temporarily.  That’s it.  No healing will be done by an epidural procedure.

The bad news is two fold:

  1. If fools your body your better or “healed” and you continue with harmful activity and damage the disc further….much further.  Quite similar to the affect of oral medications.
  2. Some feel that too many epidural injections can actually damage the outer shell or “annular” portion of the disc and thus weaken it further.

Both of these downside affects will often lead to significantly more serious issues and could possibly lead to permenant negative changes in the disc tissue.

So, only engage in these procedures with great caution and only if absolutely necessary.  I have a pain management doc as part of my team and these procedures are applied very judiciously for our patients.

Easily 75% of the patients who enter my office looking for relief have been on an extensive array of drugs… both prescribed and over-the-counter… And many have had multiple pain management procedures including epidiruals, nerve injections, electro or thermal “melting” of the disc (known as IDET) and the like.  Some have had 15 spinal injections or more … without significant long term relief.

However, in my opinion, the waste of time and your own or your insurance companies money is not the worst of the issues here.

The most serious issue at hand is clearly:

DELAY OF APPROPRIATE CARE AND FURTHER DAMAGE TO THE DISCS AND SURROUNDING TISSUES.

What do I mean by this?  Simply put, by masking the pain caused by the underlying damage, the patient is deceived feeling his “problem” has been solved.  But all that really happened was that the pain was artificially masked.  The patient continues to engage in activity that is damaging the delicate, pain sensitive tissue even more… but doesn’t know it because the pain killing medication either swallowed or ingested is acting as a cover-up.

Brief example:  70 something year old Craig came to see me with pain that had been progressing rapidly for 3 to 6 months but had started years before.  He had been actively engaged in tennis, mountain climbing, golfing and biking…but could only due so after downing some type of pain killer.  While he killed the pain, he was quietly destroying his discs. Then one day years later, the pain killers stopped working.

And he showed up at my door step in serious pain… and more serious disrepair.  MRI studies revealed massive degenerative decay at every level from the lower thoracic to lower lumber areas with multiple herniations and areas of instability.  He was as serious of a case as I’ve seen and I categorized him at a level 5 requiring the most serious and intensive care program I have available.

While Craig is slowly repairing and feeling somewhat better, he’ll never do better than 50 to 60% improvement.  Why?  Because he was kidding himself that drugs were making the “problem” better.  Truth is: it was the worst thing he could have done and has caused himself permanent and irreparable harm.  The tissues can never truly heal at this point.

Do yourself a favor.  Don’t kid yourself that drugs heal you. They don’t.  And there are other options.

I’ll explore more on epidurals in my next posting and how they should be used more judiciously and in combination with non-invasive rehab procedures.

What The Medical World And Physicians Are Saying About Non-Surgical Decompression

Are All Decompression Clinics Created Equal Or What To Look For When Choosing A Decompression Center? Part II

decomclinic2A discussion about decompression technology would be appropriate here.   Which is best and why is a question I often here.  So, let me say this right off the bat.  Some clinics are advertising decompression but use machines that are more like old fashioned traction machines.  Would you like your surgeon to be using 75 year old equipment and technology or the newest and latest. As noted by Dr. Alan Dyer MD of VaxD Technologies and former assistant health administrator of Ontario, Canada”:

“ Traction has not been shown to lower intradiscal pressures (a key to effective disc decompression) and has had dismal track record with chronic low back pain.  Traction can be expected to increase intradiscal pressures and , therefore, aggravate a protruded, herniated or extruded disc.  It is therefore, conta-indicated for patients with herniated discs.”

Therefore, the clinic you choose should have decompression technology…not old fashioned traction.  True decompression equipment is armed with state-of-of-the-art biofeedback that monitors decompression pressures multiple times per second (and make sure the patient is totally relaxed and pain-free), the most advanced software to apply decompression forces and rest cycles, ability to decompress in the face down or face up position or three dimensions, friction free application technology  to “de-weight” the area being decompressed as well as other vital factors.  If it can’t do all this, it’s probably traction…not good or effective for herniated disc type problems.  Ask what the clinic has available when you go for your initial consultation.

One more important point.

We have 4 different types of decompression technology available in our clinic.  One size does not fit all.  Most “decompression” clinics have just one (hopefully it’s not just traction) limiting the scope of patient type, size and difficulty that they can handle. In our office we use the newest in VaxD, DRX, Spinetronics 3-D antaligictrak and ABS decompression equipment.

Think about this.  Would you want to buy a car from a dealer who had only one color, one model and no options.  I think not.   Therefore,   top level facility must offer and have available more than just one single approach or technology for the best outcomes for the most difficult cases.

Post decompression three dimension rehab is also vital but we’ll get to that in another posting.

Are All Decompression Clinics Created Equal Or What To Look For When Choosing A Decompression Center? Part I

For quite awhile now, I’ve been conducting several consultations each week with severe pain sufferers who have already “treated” with… and failed… other centers offering some sort ofdecomclinic1 decompression program.  When I ask why they would visit with me after failing “decompression” procedures they would all respond in a similar fashion something like this …. “I sensed decompression should work for me but wasn’t.  My friend/doctor/one of our other patients told me about your clinic and it sounded almost totally different from the care I was receiving at the other place”.

So here’s a quick list of what to look for before agreeing to a starting a decompression program:

  1. Decompression therapy should last between 25-40 minutes per session..  Non- Surgical Decompression CANNOT work with less than adequate number of cycles per session.  (not 10 or 15 minutes like many clinics are doing)
  2. Rest time during the decompression treatment is as important as the actual decompression time.
  3. The amount of force should start at no more than 40 pounds for most woman and 50 pounds for most men.  As treatment progresses, I rarely go over 75-80 pounds regardless of the patient’s size including the huge professional and high level athletes and bodybuilders that have treated with me.   You should barely sense anything is happening when on the treatment.  If you’re muscles are sensing the decompression and contracting it will definitely nullify any positive affects of the decompression and may even injure the discs further.(not half the body weight or other such nonsense)
  4. Decompression bracing must be applied appropriately as what goes on in between sessions can seriously affect your outcome to treatment. (varies patient to patient)
  5. Ask the clinic director or doctor:  Will a doc who is a decompression expert be monitoring my care visit to visit?   It cannot be a physical therapist, tech or any one who is not highly trained in the details of proper decompression application.  It is not simply getting strapped on a table by a tech and lying there for a few minutes.  Decompression is delicate balance of high level technology and artful application of that technology.

Home Remedies for What To Do When You’re Herniated Disc, Neck Or Back Pain Flairs Up At Home And You Need Relief

Chronic sufferers of spinal pain may have periods of feeling better ….. and feeling a lot worse.  There’s a few things you can do (and things you should NOT do) at home for those bad days:

  • Get immediate application of ice packs to the area in pain.  You can also ice the area where the pain is radiating such as if your neck pain is going into the shoulder or upper back, the ice both the neck and upper back area.
  • Gel packs are better than ice cubes in a plastic bag as the gel pack will contour to the natural curves of the body and are more comfortable to use.
  • Keep the ice on 15 to 20 minutes per application and no more.  It will have the reverse
  • affect if left on too long.
  • Repeat these applications hourly.  You can do this on and off procedure all day without harm.  Repetition is key to reducing inflammation.
  • Keep a thin towel over the ice.  Don’t take it so cold so as to burn the skin.

DO  NOT   APPLY  HEAT   IN   ANY     FORM

  • Get off your feet.    The least “compressive” or stressful position to the disc and joints of the the spine is recumbent.   There is no ONE correct way to be positioned.  The one that is most comfortable for you is telling your body it is correct.

Most low back sufferers seem to feel better face up with a few firm pillows under the knees whereby the thighs are 45 to 90 degrees to the surface upon which you are lying

  • Do NOT try to exercise the pain away.  Exercise is compressive and damaging to the disc.  While quite important in a comprehensive spine rehab program, exercise should not be done in the inflammatory stage and certainly not for acute flare ups.
  • Medication:    OTC ant- inflammatories such as Alleve may “take the edge”   off.  However, any drug may be harmful to the liver or kidney if overused.   Keep it to a minimum where possible.

Special Note:  Drugs may artificially “fool” the body that your are healed.  Many will overdue it with activity and ignore the underlying damage as these drugs mask the pain and further damage the discs and underlying joints (much more on  this in another posting)

This should be helpful for short term relief during those rough times.   If you’re pain continues, seek professional consultation from a non surgical decompression specialist.

Can non – surgical decompression help with conditions like Sciatica.?  The answer is this:  If evaluation reveals you have a compression injury such as bulge, herniation or protrusion on an MRI and your doctor determines this to be the cause of your sciatic pain……. Then there is a very good chance that Non Surgical Spinal Decompression could be the answer to your problem.

As noted in PART I, when the disc gets squished, the internal disc material probes outward thus compressing or irritating the adjacent local nerves which go down your legs.legpain2

Non Surgical Decompression (NSSD), in the vast majority of cases, can reverse the high intra-disc pressure drawing the bulging disc centrally while drawing in vital nutrients and fluids which will help heal the disc in the longer run.  When successful, the pressure and inflammatory chemical reaction is eased…. and your sciatic pain goes away. Without medications, invasive epidural injections, routing physical therapy or surgical interventions.

Basically, decompression technology “fools mother nature” sneaking under the muscle reactions and spasms and opens up the disc space, creates the negative pressure and heals the disc.   The healing is usually long term and most patients do not require endless treatments or even maintenance once they have gone through the program.   Some people fell better almost immediately in a couple of sessions while for others the procedure takes a longer usually due to the severity of the underlying damage.

This affect of’ “negative pressure” has been proven repetitively in setting whereby a herniated disc patient has electronic pressure sensors inserted inside the disc.  The internal disc pressure drops dramatically once the proper thresh\hold is reached creating a vacuum like phenomena which will begin to draw in fluids to the interior of the ‘squshed” disc.

In time, most of the worst sciatica cases feel much, much better and are healed in the long term with proper application of Non Surgical Decompression procedures.  The procedure is relaxing, easy for the patient and painless.

DoctorHave you or someone you know ever had the experience of going to your doc because you had some god-awful pain from your low back or hip down into your leg or foot?   If so, you know it can range anywhere from modestly unpleasant to down right agonizing.  And if you’ve had this rather negative experience you want pain relief fast…..and that’s why you went to see your doctor in the first place.

Usually, he or she tells you it’s Sciatica.  Well, the truth is… you just been told what you already knew because sciatica literally means “leg pain”. Sort of like going to the doc because you’ve lost your vision and you’re told you’re blind.

Your concern, however, is not to get a description of what you already knew but to find out what’s causing it…. and how to fix it.

In most cases, not all but most, leg pain radiating from the hip or back is related to pressure on the nerves in your lower back.   And often that pressure comes from the jelly donut like disc that ‘s being “squished” between to vertebrae.  This squeezing pressure causes the discs internal contents to bulge or spill out creating an outward bulging of the disc which can directly place pressure on the nerves which run down into your legs.  Additionally,  inflammatory chemicals may be released which further irritate the nerve and your pain.

Unfortunately, with time, things like Sciatica tend to progress.  The outer wall of the disc begins to get tiny cracks and crevices… like a broken windshield.   The disc can decay further and lose more and more fluid thus becoming dehydrated.

Standard care may utilize prescription of anti-inflammatory medication, over the counter analgesics or pain killers.  The doc will often refer you for a course of routine physical therapy or chiropractic care.  Later, an lumbar MRI may be taken which can reveal the damaged discs.

Continued pain leads you to a pain management office for invasive injections or even the neuro-surgeon’s office for consultation.

While all these things may create some short term relief…they all fall short in the long run offering no true repair or reversal of the process.  Surgical intervention is laden with possible side affects as well as anesthesia and infection risk not to mention surgical failure or even error.

What  has emerged over the last decade or so are advanced  non surgical methods used to relieve disc pressure and actually get the disc to heal long term.  This known as non surgical decompression and we will continue this discussion in my next blog posting.
